New and emerging therapies for inflammatory bowel disease.

چکیده

SD One of the major discoveries in Crohn’s disease over the past few years has been the realization that the innate immune system plays a key role in the disease mechanism. Following an initial defect in the innate immune system, there is overreaction of the adaptive immune system, which leads to the classical inflammatory response that has been described in these patients. Ulcerative colitis has classically been described as a disease involving T helper 2 cells and increased levels of interleukin (IL)-13, which seems to be a key cytokine, but the pathophysiology of ulcerative colitis has not been studied as much as that of Crohn’s disease. Finally, another concept that is very relevant is that most diseases are dependent not only on T cells but also on nonimmune cells. While a primary defect in epithelial cells seems to be a key step in the disease process, the development of inflammatory bowel disease (IBD) also depends on the immune response and on nonimmune cells such as fibroblasts and endothelial cells.

Application of extracellular vesicles in the treatment of inflammatory bowel disease

Introduction: Inflammatory bowel disease(IBD) is caused by genetic, environmental, microbial and immune factors. IBD has two primary forms: Ulcerative colitis and Crohn´s disease. The incidence of IBD has significantly increased over the last few decades. Given that patients have poor response to drug treatments or are resistant to drug therapies, new therapies are needed for gastrointestinal i...

Effects of Probiotics and Prebiotics in Inflammatory Bowel Disease

Inflammatory bowel diseases (IBD) including most important forms; Crohn’s disease (CD) and ulcerative colitis (UC); are chronic diseases affecting the gastrointestinal tract with unclear etiology and pathology. The role of gut microbiota has recently been considered as major a factor, since altered microbiome or "dysbiosis" is a key player in the protracted course of inflammation in IBD. Howeve...

Small therapeutic molecules for the treatment of inflammatory bowel disease.

New therapies for inflammatory bowel disease are needed, because standard therapies fail to induce remission in about 30% of patients, and because of the relative inefficacy of current maintenance therapies. This review summarises the current status of the development of small therapeutic molecules for inflammatory bowel disease.

Current and emerging drugs for the treatment of inflammatory bowel disease

During the last decade a large number of biological agents against tumor necrosis factor-α (TNF-α), as well as many biochemical substances and molecules specifically for the medical treatment of patients with inflammatory bowel disease (IBD), have been developed. This enormous progress was a consequence of the significant advances in biotechnology along with the increased knowledge of the under...

New and emerging treatments for ulcerative colitis: a focus on vedolizumab

Ulcerative colitis (UC) is the most common form of chronic inflammatory bowel disease and lacks a curative pharmacological treatment. Traditional therapies, which include 5-aminosalicylates, corticosteroids and immunosuppressants, aim to induce and maintain remission. Biological therapy with anti-tumor necrosis factor-alpha agents has added to the range of available treatments. Vedolizumab belo...

Emerging Drug Therapies for Heart Failure

Heart failure is associated with high morbidity and mortality and is proving to be an economic burden in developing countries. A number of therapeutic agents are presently employed in heart failure; but they are not sufficient to control symptoms of heart failure. Moreover, the prevalence of chronic heart failure is progressively increasing and thus there is a continuing need to develop effecti...
